You are working a busy shift in your department’s fast track area and sign up for a patient with a complaint of “eye pain.” The patient is a fan of glue-on nails and mistook her nail glue bottle for her eyedrops. Now she is unable to open her eyelid for the exam and you struggle to open it yourself. You want to avoid cutting the adhered eyelashes and wonder if there’s a better solution.

Artificial acrylic or “press-on” nails were first invented in the 1950s; however, they did not gain in popularity until the 1980s as nail art became a trend [2]. Shortly thereafter, they fell out of style until a resurgence occurred with the COVID pandemic forcing the closure of nail salons. Compared to pre-pandemic values, Google searches for “press on nails” increased 300% [3]. Application of most nails requires nail glue, which often contains a mixture of alcohol, cyanoacrylate (superglue), or photo-bonded methacrylate [4].

Use of nail glue at home can result in adverse exposures with the most common location being the eye [5]. Because nail glue is often packaged in small containers identical to eye drop bottles, patients can mistake the nail glue for ophthalmic drops — especially those with visual impairment [6]. This exposure was first described in the medical literature in 1982 and has been described many times since despite repeated calls for manufacturers to modify the bottles to be safer [1].

In the presence of water, cyanoacrylate rapidly polymerizes, leading to the bonding effect [5]. If the glue gets into a person’s eye, reflexive blinking pushes the glue to the eyelid margins resulting in the eyelashes or eyelid margins sticking together [5], also known as inadvertent tarsorrhaphy. Methods to open the eyelids include removal of glue with forceps, removal or cutting of the eyelashes, or soaking the eye for hours to days with a moist gauze [1, 6, 7].

Although the most successful solvent to dissolve dried glue is acetone, this can cause corneal and conjunctival injuries [8]. The effectiveness of other solvents has been debated in the literature with mixed reports of efficacy [9].

Our personal experience managing several of these cases suggests that a petroleum-based topical antibiotic ointment, such as Bacitracin, can help loosen the glue bond. It is an inexpensive option with minimal harm to fix inadvertent eyelid adhesion from nail glue (or other superglue). It is worth trying before attempting more aggressive techniques.A recent news report about a woman who accidentally glued one of her eyes shut when she mistook Super Glue (cyanoacrylate adhesive) for her eye drops is a reminder that the potential for this mix-up is real. The Associated Press reported that a woman who had cataract surgery a year ago was reaching for what she thought was one of her half-dozen eye medications but picked up a nearby super glue container in error. A burning sensation immediately indicated that something was seriously wrong, so she went to the hospital where doctors worked on getting her eye open.

This is certainly not the first time this sort of incident has occurred. In fact, ISMP previously wrote about a nearly identical incident that happened to a New Jersey woman. She too instilled super glue (cyanoacrylate) into one of her eyes and glued them together. A search of the medical literature confirms that this has happened numerous times.

There are many cyanoacrylate adhesive products and they are known by many different brand names. At one time, all of these glues did come in containers that were nearly identical to eye drop containers, but problems happened so often that changes were made to reduce potential confusion. Today, many types of eye glue are sold in a tube shape container, however, some can still be found in containers similar to eye drops. Still, patients with poor eyesight might be at risk of potentially mixing up these products, even with the newer container designs. For example, Super Glue looks somewhat like a tube of eye ointment.

The bond formed by these glues is extremely strong. In fact anyone who’s used cyanoacrylates and gotten glue on themselves has probably run into a situation where their fingers got stuck together or to whatever was being glued, and it became difficult to pull things apart. This is such a common problem that the manufacturer of Super Glue, the Super Glue Corporation, has a page on their website devoted to removal of the glue from body parts. As for fingers sticking together or sticking to whatever you’re gluing, that’s an easy to fix. A small amount of nail polish remover applied with a Q-tip will do the trick. Be sure the nail polish remover contains acetone, a mortal enemy of cyanoacrylate glues. You will not want to use acetone near your eyes though, since that could be damaging to vision.

You're standing at the Starbucks counter, about to pay for your overpriced but oh-so-good morning latte. You reach into your bag to fetch your purse, grab your money and pay the cute barista behind the counter.  As you hand over the cash and flash a subtle grin his way, your eyes drift down to the area your hands are in and are greeted by a single stubby, flesh-coloured nub, nestled in amongst your perfectly manicured talons.  You hurriedly thrust your hand into your pocket, grab your coffee with the other hand, and scurry out of there, cursing your nail glue under your breath as you go.  It's a familiar scenario for most press-on nail wearers, whether it's actually happened to you, or just something you've experienced in your nightmares.

Unfortunately, there's no way to make your nails stay on indefinitely.  No nail extension can ever be truly permanent, (and how boring would that be, anyway.) But if you follow the tips below, it will certainly put some distance between you and the nightmare scenario above!

If you don't have a prep wipe, using pure isopropyl alcohol or pure acetone will also work.  (Don't use nail polish remover, though, even if it's acetone-based. This will have some kind of moisturising agent in it which does the exact opposite.)

This one is kind of tied in with number one, but pushing back and removing your excess cuticle is always a good idea.

It's surprising just how far down your nail your cuticle can grow, even if you can't see it at first.  To check this, I like to gently run the pointy end of an orange wood stick around my nail, just below the live part of my cuticle. (That's the bit that hurts if you jab it) You'll be surprised how much old dead cuticle starts lifting away.

Removing this excess cuticle is actually really easy, and you don't even need to cut them! With an orange wood stick and a bottle of cuticle remover, it only takes about 5-10 minutes, and doing this once a fortnight should be enough to keep them well maintained.  Just apply the remover to your cuticles, and let it sit for the time specified by the maker (usually a minute or so.)  Then use the flat side of the stick to gently push your cuticles back towards your body.  This should be gentle and completely painless, so if it hurts even a little bit, you're pushing too hard.  You'll notice little blobs of milky gel appearing as you do this, so you'll want to wipe your stick off on some tissue now and then.  Once you've pushed back all of your cuticles, wash your hands thoroughly to remove all trace of the product, trim any hangnails with cuticle nippers, and you're good to go.  

You can get liquid/gel cuticle removers from many different brands, but my favourite is Sally Hansen's Instant Cuticle Remover. (a blue/aqua coloured gel - pictured above) It's widely available, very effective, and the gel formula and small nozzle make it very easy to apply.  

 but the longer you can leave it after just applying your nails, the more chance your adhesive will have to fully bond.  Rubber gloves and hand sanitizer can be powerful allies here.
